ALL APPLICANTS MUST COMPLETE AN IMMUNIZATION FORM BEFORE ENROLLING AT NORTHWESTERN. This form is available on our website. Verfication of MMR and Hepatitis B vaccinations is also required.

Print the application agreement statement that appears after you submit the application.
The nonrefundable fee is $15 (Visa, MasterCard, or Discover Card).

The following documents are required to process this application:
- New freshman student
- Official high school transcript with final GPA, rank and number in class, graduation date (GED scores or equivalent)
- ACT/SAT scores
- New transfer student
- Complete official college transcript from all institutions attended showing degrees received
- If less than 24 college-level hours earned, high school transcript and ACT/SAT scores
- High school concurrent student
- Current high school transcript with current GPA, rank, and number in class
- ACT/SAT scores
- Signed concurrent student permission form
- New graduate student
- Complete official college transcripts from all institutions attended showing good standing and degrees received

Documents Retention:
All documents submitted to Northwestern Oklahoma State University for admission purposes become the property of the university.
Documents will not be released to students nor will they be forwarded to other educational institutions or agencies.
Directory Information
In accordance with the provisions of the Family Educational Rights and Privacy Act of 1974, if you DO NOT want directory information about yourself given to a third party you MUST fill out a Request to Withhold Directory Information form and return it to the registry office.
Directory information includes name, address (current and permanent), birth date, telephone number, hometown city and state, verification of current enrollment, dates of enrollment, degrees conferred and dates, major field of study and honors, previous schools attended, hometown newspaper, and in participation officially recognized activities and sports.
